I go straight up and down - others do more of a cross-over technique - but as long as there is resistance to dorsiflexion, I think this style works wayyyyy better than side-to-side buddy splinting. Well, it was a high dose. But the nail was the issue - it needed to come off. Good question tho. TY
did you use the same scalpel to cut the flaps as you did to cut the wart out? if so, doesn't that risk the other sites getting warts?
Great question. But the virus is EVERYWHERE. I've done simple full thickness excisions (no flap) and they've still come backl
I remember I had one of these as a kid, in pretty much the exact same location. When you have a verruca, you can feel it is deep by the pain you get. Well I did the same surgery as here, but instead of getting the blade under it to cut it out, I clenched my teeth and ripped it out. It hurt a bit. There was a fair amount of blood, but after that, the pain of walking on the the verruca instantly went away. It was literally gone. I was shocked at how big it was under the skin. I did examine it, and noticed small spikes at the bottom of it. It never came back.
I had k wire removal on four toes on Friday.. my toes won’t bend.. how long will it take for them to bend or will they never bend again? I had bone removal at the knuckle/shortening procedure.. scared my foot will never be the same..

Why isn't a Tourniquet being used to control the bleed during the procedure. Most podiatricst use one in order to see better.

Hello Doctor, I am sorry to contact you like that, but I couldn't find another way. I have ingrown toenails on both big toes and the second toe of my left leg. In the country I stay, India, we usually consult a general surgeon or a plastic surgeon for the procedure since finding a podiatrist is rare. I consulted two plastic surgeons and they both have different approaches to treating my nails. 1. The first doctor said he would remove the ingrowing part of the nail, along with the germinal matrix and the sterile matrix after which I will be sent home. He will also be giving me stitches. 2. The second doctor on the other hand said he would remove the ingrowing part of the nail, and apply phenol to prevent recurrence. Both doctors are qualified and experienced which makes it difficult for me to choose. Based on your experience, which procedure is better and will give me a permanent solution?
Phenol is the BETTER technique on the whole, but both work. Note this is GENERAL advice, and I cannot say more than that for your specific case ...

Wanted to ask, my grandpa has one exactly like this and we have seen the doctor who tried to trim it but its causing him huge pain. Hes on blood thinners and type two diabetic so my question is do you think he could have his removed in the right setting ? Hes only seen the local dr about it but if we took him to a specialist like yourself is there a way they can remove it and limit the bleeding afterwards? Maybe in a hospital setting or something. He is super tough never takes painkillers even with having kidney stones/ shingles etc but this is really bothering him to the point the GP has prescribed strong pain meds and he's actually taking them which is very very rare for him. Hello Dr Reilly, have you performed this hydrodissection technique for medial nerve neuropraxia (Jogger's foot) ? I got medial plantar nerve entrapped or somehow is being compressed within the knot of henry when walking/standing. All conservative treatments didn't fix it and all tests showed OK, except EMG test: which only showed a minor degree axonal damage to medial plantar nerve. Thank you

@@IanReillyPodsurgeon Thank you for the reply. Therefore, does it work as a nerve release ? Can it be a permanent solution then ? I have this pain for almost 2 years, it is like plantar fasciitis. It looks like the nerve cannot be heal by itself due to be compressed/pinched and it is exactly in the medial nerve branches (knot of henry).

I had stitches for both my big toe ingrowing toe nails in the early 1980's on the NHS afterwards a week or so later a local doctor tried to pull the stitches out which where at the bottom a cm from where the nail starts it was too painful so guess what they never bothered again and I had them stitches in for 25 years plus eventually I pulled them out myself!

Hi Dr , quick follow up, I had my k wire remove from the same toe last week (4th toe) and had a check up today with x ray (6 weeks after I broke it) I’ve been told healing well but continue to wear my boot for another 2/3 weeks, is this normal? I was suspecting to be told I was back to normal today
i cant comment on YOUR case, but I tell MY patients that toes take 6 months to settle.
I am very fortunate to work in one of the hospitals Mr Reilly operates in. The man is a true legend and a wonderful colleague.

This is one of the toughest injuries to treat both conservatively and surgically. Great technique, I normally supplement with some at home rehab trying to strengthen the flexors and maybe an orthosis. I can't tell you how many times I've seen these injected with cortisone by other providers (sometimes thinking it's a "neuroma") with subsequent complete rupture and dislocation of the toe. I think injecting this area is like injecting the achilles - no man land.
@@districtfootandankle totes. I maybe put a tiny smidge of steroid if it is stable and very sore. But I too have seen ruptures. Two of my own before I learnt not to. Now just from St elsewhere.

multumesc! Yes - phenol is the way to go most times. I use these for revisions and for quicker healing

I have a wire in my left big toe and I still have about 4 weeks or longer to go. Mine is bent but I am allergic to nickel. I think my Dr had a titanium one that was bent I'm not sure lol. I just asked my Dr yesterday how he will remove it and he said he'll pull it out and it shouldn't hurt at all. I can't wait to walk normally again ❤ it's been over 2 1/2yrs since I've had foot trouble.
